Supporting Patients through Cancer Diagnosis and Treatments in the Medical Home

In a partnership between the PCPCC and the American Cancer Society, the PCPCC developed three fact sheets that aim to help patients with cancer better communicate with their primary care team and oncology team during initial diagnosis, during treatment and as survivors of cancer. Specifically, these fact sheets are designed to help patients understand the basic roles of the medical home specific to cancer prevention, treatment and survivorship. The fact sheets provide patients with a set of key questions they can ask to help define the roles of each member of the care team. At the heart of the project is the desire to empower patients to ask open-ended questions of their primary care team, so that patients and providers understand and agree on expectations and responsibilities.

This partnership also includes a three-part webinar series in December 2013.

Webinar: Working with Care Teams During Treatment
Thursday, December 12th l  1 PM - 2 PM ET

Once you are done with cancer treatments, your Medical Home Team will handle most of your care. You will still need to see your Oncology Team for routine follow- up care. Make sure your Medical Home Team gets the data from these visits. This checklist will help you work with them to stay as well as possible after your treatment.
