Older Adults' Health

As the American population continues to age, the priorities of the healthcare system must be readjusted. The National Council on Aging states that about 80% of older adults have at least one chronic condition. Primary care clinicians a unique opportunity to provide these elderly patients with particularly effective have comprehensive care. Ensuring that older adults have their health needs taken care of often requires the entire health team, making care coordination even more important. Elderly patients with chronic conditions, especially if they have more than one, benefit greatly from care that is simple and streamlined. Having a primary care clinician and team that are able to provide comprehensive, patient centered care is essential in ensuring that this population receives the quality care they need and deserve.
