PCPCC’s  2012 review of 46 medical home initiatives throughout the U.S. provides significant evidence that the medical home model reduces overall costs and spending by reducing inpatient visits, emergency department use, hospital readmissions, and other factors.

This program from the American College of Physicians contains self-paced on a variety of operational and clinical areas that contain background information, the ACP Practice Biopsy, and links to a Resource Library. In addition, the program also inclues user-submitted best practice videos.
