Naval Hospital Jacksonville Medical Home Port puts patients first

Medical Home Port is Navy Medicine’s approach to the nationwide “medical home” model of care.

Patients with a primary care manager (PCM) at the one of its six facilities belong to a Medical Home Port team that places the patient at the center of a collaborative team of caregivers — from doctors to case managers.  

Medical Home Port teams focus on meeting all urgent, preventive and routine health care needs and work together with the patient to coordinate a whole-person approach to health. There are 14 Medical Home Port teams throughout the Naval Hospital (NH) Jacksonville enterprise — its hospital and five naval branch health clinics (NBHC) in Florida (Jacksonville, Key West and Mayport) and Georgia (Albany and Kings Bay). More than 71,000 active and retired sailors, soldiers, Marines, airmen, guardsmen and their families are enrolled with a PCM and Medical Home Port team at one of these facilities.

To schedule an appointment with a PCM, patients should call the appointment line at 904-542-4677 (7 a.m. – 4 p.m. for patients assigned to the hospital), or 904-546-7094 (6:30 a.m. – 4 p.m. for patients assigned to NBHC Jacksonville — active duty only,). Primary care clinics are open longer at the hospital to better serve patients and offer appointment times when needed.

At the hospital, Medical Home Port clinic hours (Family Medicine, Internal Medicine and Pediatrics) are Monday – Thursday, 7:30 a.m. – 7 p.m.; and Friday, 7:30 a.m. – 4:30 p.m. For patients assigned to NBHC Jacksonville, Medical Home Port clinic hours are Monday – Friday, 7 a.m. – 4 p.m.

Included in the Medical Home Port experience is RelayHealth, a free and secure email service that allows patients to contact assigned PCM for routine needs. Because it’s for non-urgent issues, it can take up to one business day for a reply. Also included in the Medical Home Port experience is the Nurse Advise Line — a 24/7 after-hours portal for clinical advise. Registered cnurses are available to assess symptoms, assist with self-care, and advice about children’s medical issues.
