TransforMED Convenes Patient-Centered Medical Neighborhood

Kickoff Learning Collaboratives with Health Care Organizations in Seven Communities

Health care organizations in seven U.S. communities this month begin their transformation to a Patient-Centered Medical Neighborhood (PCMN) concept of care at kickoff learning collaboratives led by TransforMED, a nonprofit subsidiary of the American Academy of Family Physicians.The seven organizations are the first of 15 health systems nationwide to initiate TransforMED training as part of the CMS Center for Medicare and Medicaid Innovation’s PCMN demonstration project. The organizations kicking off their training programs in January are:    

  • Charleston Area Medical Center, Charleston, W.Va.   
  • Columbus Regional Hospital, Columbus, Ind.   
  • Huntsville Hospital, Huntsville, Ala.   
  • Northeast Georgia Health System, Gainesville, Ga.   
  • North Mississippi Health Services, Tupelo, Miss.   
  • Greater Baltimore Medical Center, Baltimore   
  • INTEGRIS Health, Oklahoma City
