Maine has had an impressive history and continued commitment to improve the health of its citizens through strengthening its system of primary care across the State. In the fall of 2007, the Maine Legislature convened the Commission to Study Primary Care Practice to examine the issues facing primary care and ways to stabilize and support it. In the 2008-2009 State Health Plan, the Governor’s Office for Health Policy and Finance identified the need to promote primary care as the foundation for our state’s health system.

Formed in 2003 and incorporated in 2006, Maine Quality Counts (QC) has provided leadership, advocacy and support for improving primary care in partnership with the State through several multi-stakeholder initiatives including PCMH learning collaboratives, a Multi-payer Advanced Primary Care Practice program, Coordinated Care Teams, Quality Counts for Kids, and Integrated Behavioral-Physical Health, to name a few. Maine Quality Counts, as one of the State’s designated partners in the State Innovation Model (SIM) Initiative, is working to ensure all State health improvement measures build upon the success of these and other established PCMH programs. Quality Counts is leading the learning collaborative to implement the new Maine Behavioral Health Home Initiative.

Cigna Collaborative Accountable Care ACO - Beacon Health

Cigna (NYSE: CI) and Beacon Health , a subsidiary of EMHS (Eastern Maine Healthcare Systems), have launched a collaborative accountable care  initiative to improve patient accessto health care, enhance care coordination and achieve the “triple aim” of improved health, affordability and patient experience. The program became effective January 1, 2014.

Maine Shifts Health Focus To Community

For the last three of the nearly six decades Shirley Estes, 80, has been married to her husband Harry, since his first heart attack and quadruple bypass at age 51, she has spent so many nights and early mornings in the Eastern Maine Medical Center emergency room she has lost count. Most of the time, Shirley handled her husband's unpredictable heart health by herself. Last summer, she laid slacks and a blouse on her bed each night so she could quickly change out of her nightclothes and rush her 81-year-old, ill husband to the hospital, a few miles away in Bangor.

Beacon, LLC - CMS Pioneer ACO

Beacon Health, a subsidiary of EMHS, oversees efforts around EMHS’ Pioneer Accountable Care Organization (ACO). The program has demonstrated improvement in care coordination, efficiency, effectiveness, and quality all while reducing costs.  The Centers for Medicare and Medicaid Services (CMS) announced Beacon Health had a shared savings for performance year one of nearly 5%.

MaineCare's Health Homes Initiative

Maine has three Health Homes programs, the first is their traditional chronic conditions HH. On January 17, 2013, the Centers for Medicare & Medicaid Services (CMS) approved a Section 2703 health home State Plan Amendment for Medicaid enrollees with chronic conditions that is closely tied to the state's Patient-Centered Medical Home pilot.

Aetna Accountable Care - Maine

Aetna announced five accountable care agreements in Maine. The agreements align payment for medical services at major health systems in Maine with improved quality, efficiency and patient experiences.The agreements support Aetna's commitment to develop accountable care models for the State of Maine's employees and their dependents, which Aetna administers.
