About Us

The Primary Care Collaborative (PCC) is a not-for-profit multi-stakeholder membership organization dedicated to advancing an effective and efficient health system built on a strong foundation of primary care and the patient-centered medical home. Representing a broad group of public and private organizations, the PCC’s mission is to unify and engage diverse stakeholders in promoting policies and sharing best practices that support the growth of high-performing primary care that treats the whole person. We advocate for comprehensive and integrated health care, delivered by a team, to achieve the “Quintuple Aim”: better care, better health, lower costs,  greater joy for clinicians and staff, and greater health equity.
The PCC was founded in 2006 as the Patient-Centered Primary Care Collaborative (PCPCC, an acronym you'll still see on various places on our website, including in our website address and social media channels). Its name was shortened to the Primary Care Collaborative (PCC) in 2019, although the organization still focuses on the benefits of high-performing primary care for patients.

Mission

The Primary Care Collaborative advances comprehensive primary care to improve health and health care for patients and their families by convening and uniting stakeholders around research, care delivery and payment models, and policies.

What we do

The PCC brings together physician and nursing groups, patient and consumer organizations, health plans, employers, industry, quality organizations, health systems and many more to forge consensus around policy solutions that further primary care.
