Improving Patient/Family Engagement Through Peer Support for Parents of Children with Special Needs

Engaging patients and families to be partners with their healthcare team is of increasing focus across the healthcare system. Strengthening this collaboration has been shown to enhance a person’s ability to manage their care, increase communication between people and their healthcare team, and improve the delivery of healthcare overall. For parents of children with special needs, this engagement empowers parents to access necessary support and care their children need. During this webinar, participants will learn about Parent to Parent, a national per support program for parents/families caring for children with special needs and how peer support strengthens and accelerates adoption of key patient/family engagement strategies important for achieving aims of the CMS Transforming Clinical Practice initiative.

If you are interested in learning more about how peer support can help your practice, please join us at Virtual Peer Support Meeting (PFCC.Connect) on September 27, 2018, at 1:30 PM EST by registering here.
