How Meaningful Patient Engagement Drives Value & Shared Vision for Change

The PCPCC SAN and HealthTeamWorks have joined forces to present a webinar that will focus on meaningful patient engagement in transformation using a blended model to inform and improve progress toward the Quadruple Aim. Patient experience surveys at the corporate and practice level provide quantitative patient experience results that are helpful to leadership teams. By taking additional steps to engage patients at the practice level in quality improvement, richer and more timely qualitative data can be discovered that supplement and enhance the survey data.

Cigna Medical Group - an owned multi-specialty group in Arizona - went beyond surveys to capture patient feedback at both the practice and regional levels through patient focus groups. By engaging patients, practice-level providers, staff and executive leaders together in this highly interactive process, the medical group was successful in creating a shared vision for change and helped to refine and improve transformation efforts in a truly meaningful way.

Webinar objectives:

1. Define patient engagement in practice- and system-level quality improvement efforts.
2. Identify three ways to engage patients in quality improvement efforts to achieve a blended model.
3. Share a case study highlighting the benefits experienced and lessons learned by a large multi-specialty medical group in their transformation efforts.

We'll have office hours for 30 minutes after the webinar to answer questions not addressed during the presentation (2-2:30pm ET).
