The PCMH healthcare transformation model represents a unique opportunity to implement solutions to bridge the gap in healthcare disparities barriers to improving healthcare outcomes. Employers have long recognized the business case for addressing healthcare disparities in order to improve health outcomes.  Audrey Whetsell of the Medical Home Development Group will introduce a comprehensive approach to PCMH practice transformation using innovative technology solutions.  She will highlight how to seamlessly incorporate unique technology solutions sets, that can assess and address health literacy, cultural competency and language barriers,  into the PCMH transformation and accreditation process.  This enhancement in the accreditation process  will help front line providers optimize healthcare quality and outcomes for all patients.

User friendly technology solutions from the Hopkins Center for Health Disparities Solutions and Polyglot Systems will be previewed during the webinar to provide attendees with a solution oriented perspective to improving patient-provider communications and partnership. Dr. Thomas LaVeist and Cheri Wilson will demonstrate how the Cultural Competency Organizational Assessment-360 (COA360) can be successfully deployed as a continuous quality improvement (CQI) approach to improve organizational cultural competence. 

These assessments: 

  1. establish a baseline of performance
  2. identify deficiencies
  3. enable the implementation of targeted interventions
  4. determine the effectiveness of these interventions through re-assessments.

Dr. Charles Lee will present several technologies developed with grant funding from the National Institute for Minority Health and Health Disparities to eliminate disparities for patients with limited health literacy and/or language barriers.

Some of the technologies covered will include:

  1. simplified medication instructions that address health literacy
  2. video demonstrations to promote proper medication techniques
  3. personalized, actionable discharge instructions in single and dual language formats
  4. customizable, translated vital healthcare documents
  5. easy-to-follow reconciled patient medication summaries.
