UPMCHPs PCMH Program is a health care delivery system focused on patient centricity and founded on the principles of the patient-centered medical home and the Wagner Chronic Care Model. With the assistance of UPMCHP’s integrated care coordination team and practice-based care managers, physicians are better able to deliver continuous, accessible, high quality, patient-oriented primary care. Through UPMCHP’s work with PA Reach, the Practice Transformation team works with individual practice offices to achieve meaningful use certification.
