Transforming Clinical Practices (TCPI)

The Transforming Clinical Practice Initiative (TCPI) was a four year grant launched by CMS in 2015 to help practices across the United States achieve large scale transformation. TCPI provided technical assistance to more than 140,000 clinicians from 2015-2019, allowing them to further develop their quality improvement strategies. The primary goals of TCPI were to support practices enrolled in Practice Transformation Networks and Support and Alignment Networks, build a transformation evidence base to help spread the effective solutions, and improve health outcomes by reducing unnecessary hospitalizations and overutilization of other services. 

The PCC acted as a Support and Alignment Network under the TCPI. The PCC particularly focused on recruiting and assisting small, rural and medically underserved communities. While the TCPI grant has since ended, the momentum has sustained.
