Patient/Consumer Evaluation of PCMH

In order to evaluate, monitor, and benchmark patient satisfaction, studies are being conducted to analyze how an individual regards a health care service or product and the manner in which it is delivered by an organization, hospital, or health provider.  One of the major studies is the Consumer Assessment of Healthcare Providers and Systems (CAHPS), which is an ongoing evaluation conducted by the Agency for Healthcare Research and Quality.

The CAHPS PCMH Survey was first implemented in 2011. The survey addresses domains of care that multiple stakeholders identified as critical for evaluating PCMH practices. These domains include:

  • Access
  • Information
  • Communication
  • Coordination of care
  • Comprehensiveness
  • Self-management support and shared decision making

CAHPS PCMH Questionnaires

There are currently two CAHPS PCMH questionnaires covering the adult and child populations. The CAHPS PCMH questionnaires are produced by the Agency for Healthcare Research and Quality (AHRQ). To view a copy of the CAHPS PCMH survey, click here.
