Van Chaudhari

Van is currently the Administrator for Population Health Management at UW Medicine providing direction and structure for carrying out quality and care commitments in value based contacts for Medicare Advantage, Commercial and Medicaid populations, building community partnerships and actualizing  the patient centered medical home in primary care to achieve better population health. As a strategic and forward thinking health care leader, Van has 19 years diverse consulting, strategic and operational experience in academic and community health care settings spanning primary care clinic networks, specialty outpatient to emergency department, hospital – acute and rehab, post-acute care and community health networks. He also has experience leading and providing structure for care transformation in ACOs, clinically integrated networks, developing and implementing quality and care standardization initiatives in primary, ambulatory, acute inpatient, ED and post-acute settings.

Van has a bachelors degree in Industrial and Systems Engineering and a Masters in Health Care Administration. He also serves as the Clinical Assistant Professor at the Department of Health Services Research at University of Washington, School of Public Health.
