Marie Maes-Voreis

Marie has led the implementation of Health Care Homes legislation for the Minnesota Department of Health and the Minnesota Department of Human Services as a joint government private sector project since 2008.  In addition, she is the State’s Co-Lead for the community integration and practice transformation work for the Minnesota Accountable Health Model-SIM Minnesota testing grant. Prior to joining MDH Marie worked as a Director of Primary Communication systems at Park Nicollet Health Services in St. Louis Park, MN and the Director of the Family Medicine clinics at Hennepin County Medical Center (HCMC) in Minneapolis, MN. She managed the successful implementation of medical home using a multi-disciplinary team care model in family medicine /behavioral health and has extensive experience with implementation of the EPIC electronic health record.  Marie has a Master’s degree in Health and Human Services Administration from St. Mary’s University and a Bachelor’s degree in Nursing from the College of St. Benedict.
