A Year in Review: All Eyes on Primary Care

Dear Members and Friends, 
 
Thanks to you, primary care is in the health care spotlight as never before. As an advocate of primary care and the medical home, we thank you for your continued support of the movement, and of the Primary Care Collaborative (PCPCC). Your engagement in our collective work helps us accomplish our mission of transforming the fragmented, inefficient U.S. health system into one that is built on a strong foundation of patient-centered primary care and the medical home. We had a busy year and we're delighted to share some of our joint accomplishments with you in the newsletter below.

Looking ahead to 2015, we will finalize a new strategic action plan with guidance from our Board, Executive Members and Center leaders. We have made significant progress in sharing the growing evidence about the positive impact of patient-centered primary care and the medical home but we can do more. First, we need to align our efforts with like-minded colleagues and ramp up our policy and advocacy efforts, especially in regard to payment reform that rewards Triple Aim goals rather than the number of visits and procedures. Further, we need to promote the education, training and integration of truly team-based, interprofessional care. Finally, we need to better engage with patients and the public as critical partners in these efforts. With a new Congress and new leadership in the Senate, we want to use our collective voices to inform and educate policymakers and the public about the power of enhanced integrated primary care as the foundation for much needed health system transformation.

As you may know, nearly all of the work we do at the PCPCC is supported by your volunteer leadership and expertise, and for many of you in the medical home community, your participation as dues-paying Executive Members. If you aren’t already, we encourage you to consider joining the PCPCC as an Executive Member to play a stronger role in setting the direction for the organization - we would love to hear your ideas and energy as we move into the next phase of our movement. Thank you again for all you do.

Hosted 3 large learning & networking meetings that attracted more than 1,500 participants in Denver, Philadelphia and Washington, DC. Read our recap from the DC conference and check out photos!
 
Welcomed 29 new organizations from a broad range of organizations and industries to our Executive Membership and grew our "Center Leadership Cabinets" by 49%.
Getting the Word Out

 
Released 6 advocacy alerts on issues including Medicaid parity and the repeal of the Sustainable Growth Rate in favor of progressive payment reform. View our alerts and sign up to receive them. 
 
Held 14 educational webinars on topics critical to patient-centered primary care reaching more than 6,500 individuals. Access and watch all of the 2014 webinars in addition to all previous years archived on our website.
 
Broke our goal of 2,000 Twitter followers and reached over 1,300 Facebook fans. Stay engaged and connect with us as we spread the medical home word via social media!

5 New Resources You Can't Live Without
Primary Care Innovations and PCMH Map: Maps a diverse range of programs, payments and outcomes using medical homes and enhanced primary care teams as the model for improving health care delivery. Almost 10,000 users have accessed our map since June.

The Medical Home’s Impact on Cost & Quality: An Annual Update of the Evidence, 2012-2013: Report was cited by Medical Economics, Fierce Healthcare and many other health care news sources. Downloaded more than 6,000 times. Register for our Capitol Hill briefing on Jan. 30, 2015 to hear about our new report that will be released at the event.

Patient Centered Primary Care Training Programs Database: Contains nearly 130 unique education and training programs that prepare health professionals for delivering team-based primary care; accessed by nearly 5,000 users.

Progress and Promise: Profiles in Interprofesional HealthTraining to Deliver Patient-Centered Primary Care: A just-released publication that highlights how seven programs around the country train a variety of health care professionals to work together as teams in patient-centered medical homes. Jointly sponsored with seven national health professions organizations. 
 
Behavioral Health Integration into the PCMH Slide Decks: Two slide decks that can be downloaded and customized for presentations regarding the value of integrating behavioral health into primary care and the patient-centered medical home. Close to 2,500 downloads since October.

Speakers: PCPCC, Milbank Memorial Fund, Blue Cross Blue Shield, The Center for Medicare and Medicaid Innovation, other guest speakers to be announced.
What: Hosted by Senator Chuck Grassley (R-IA), we will release PCPCC's newest report, The Patient-Centered Medical Home's Impact on Cost and Quality, Review of Evidence 2013-2014.
When: Friday, January 30, 2015, 11:30 a.m. - 1 p.m. ET.
Where: Capitol Visitor Center, SVC 203-02, Washington, DC.
