Year in Review: A Medical Home for the Holidays!

What a year its been for primary care and the patient-centered medical home! And as a much welcomed finale, you may have heard that the Senate Finance Committee and House Ways and Means Committees both voted to approve a bipartisan, bicameral "Doc Fix" legislation last Thursday. If passed into law, it will permanently repeal the Medicare Sustainable Growth Rate – and avoid a 24% Medicare payment cut.

This signifies a major step toward moving the U.S. health care system away from a fee-for-service (FFS) payment model to one that rewards quality, efficiency and innovation. Further, it recognizes the PCMH as a framework for advancing alternative, value-based payment models.


In this year’s final (!) Week-in-Review, we're delighted to reflect on the PCPCC's many accomplishments and activities in 2013, and we couldn't have achieved it without all of your support! In January, we look forward to releasing our Annual Report (supported by the Milbank Memorial Fund), where we provide an in-depth review of 20 academic and industry studies released over the past year that demonstrate the PCMH's impact on cost, quality, and outcomes. We’ll also include a collection of highlights from more than 50 medical home studies across the 50 states since 2009.

You can hear more about the report during our Monthly National Briefing on Thursday, January 30th at 1PM, where the reports co-authors and reviewers will discuss findings, recommendations and implications for the PCMH's future. We grew...
We welcomed 20 new Executive Members from across a broad range of industries including health plans, technology, transformation or "technical assistance" organizations, national community organizations like the YMCA of the USA and the American Heart Association, plus we welcomed our first academic medicine department (University of Minnesota Dept. of Family Medicine - Go Gophers!). Plus, our General Membership increase to more than 1,100 organizations. 

We welcomed 4 new members to our Board of Directors including 
Elizabeth J. Fowler, PhD, JD, Johnson & Johnson; Jill Hummel, JD, WellPoint, Inc.; Hal C. Lawrence, III, MD, American College of Obstetricians and Gynecologists; Adrienne White-Faines, MPA, Executive Director, American Osteopathic Association.
We shared stories...
5,000 health care professionals from across a broad range of industries joined us for: Led by 50 speakers who presented on emerging topics in primary care, including nationally recognized experts like Dr. Jaan Sidorov on population health management, Dr. Art Kellerman on health IT and Dr. Kavita Patel on payment reform. 

We also kicked off a new series on eHealth Innovation Profiles including stories that highlight the critical role of Health IT in primary care transformation.

We convened...
400 medical home supporters attended our 2013 Annual Fall Conference in Bethesda, MD, where we offered 25 workshops & panels, led by nearly 60 panelists & speakers.

4,000 individuals supported the work of our Stakeholder Centers, Task Forces, and Special Interest Groups.

150 medical home stakeholders attended the 5th Annual National Medical Home Summit in Philadelphia co-hosted by the PCPCC and the Thomas Jefferson University of Population Health.
We advocated...
We submitted multiple letters to Congress in support of payment reform and the integration of primary care and mental health; and we prepared a Capitol Hill side-by-side comparison of 5 major health system transformation proposals and their implications for primary care.

Our CEO Marci Nielsen and Board President, Dr. Paul Grundy traveled across the U.S., and traveled to more than 100 local, state, and national events to articulate the PCMH's value and impact.

We submitted 31 recommendations advocating for patient and family engagement in the NCQA's PCMH recognition standards.
We shared resources...
Released a free online database that includes 100+ team-based interdisciplinary Education & Training programs and highlights innovative best practices in training the heatlh care workforce.
 
We developed our first 3-part patient support initiative in partnership with the American Cancer Society, that includes health literacy tools and webinars to support patients newly diagnosed and in treatment for cancer. 
 
We launched a population health management guide outlining our Top 10 recommended health IT tools that's already been downloaded by nearly 5,000 individuals.
