After our holiday hiatus, we are excited to kick-off 2014 with the reveal of our Annual Report next week!

On Monday, you'll have a chance to get your copy of our latest publication: The Patient-Centered Medical Home's Impact on Cost & Quality: An Annual Update of the Evidence, 2012-2013. The report is sponsored by the Milbank Memorial Fund, and I was delighted to collaborate with co-authors, Drs. Kevin Grumbach, Paul Grundy, and Nwando Olayiwola. The report presents a showcase of the year's 20 peer-reviewed and industry-generated studies that demonstrated the medical home's impact on cost, quality, and population health outcomes.

The Results Are In!
An Annual Update of Medical Home Evidence
The PCPCC will release its Annual Report next Monday, January 13th. Sponsored by the Milbank Memorial Fund, The Patient-Centered Medical Home’s Impact on Cost & Quality: An Annual Update of the Evidence, 2012-2013, analyzed quantitative outcomes from 20 medical home evaluations released over the past year. The report includes impressive results from commercial health plans, integrated health systems, academic medical centers, multi-payer initiatives, the US Military, and the Veterans Administration.

EHRs Help Physicians Improve Care
Building on reports citing the benefits of using electronic health records (EHRs), a new study from Health Services Research found that nearly 75% of physicians who use EHRs experience improvements in clinical outcomes. 

The study analyzed responses of 3,180 physicians who participated in the Physician Workflow Survey questionnaire regarding their experiences using EHRs in 2011. A majority of physicians stated that EHRs alerted them about possible medication errors, and one-third said that EHRs improved patient communication and helped identify necessary tests.

Horizon BCBSNJ Launches Patient-Centered Pediatric Program
In an effort to improve care coordination and quality outcomes for children and their families, Horizon Blue Cross Blue Shield of New Jersey announced the launch of its first patient-centered program devoted to pediatric care. The program is expected to serve more than 100,000 pediatric patients. 

Over 600 pediatricians will be rewarded for developing care plans for patients with complex care needs, demonstrating improved outcomes, and reducing avoidable emergency room visits. The program also uses outcome-specific payments to incentivize practices to achieve specific quality outcomes, including improving immunization rates and medication management.

Implementing Effective Care Management
It is well known that care management for high-risk patients is a critical component of the medical home. The American Journal of Managed Care released a study last month assessing the impact of care management on quality improvement for diabetes patients in 25 medical homes. The study found that practices with the greatest improvement in quality measures had:
  1. Strong patient-centered care manager duties
  2. An effective and actively used electronic medical record system
  3. Solid integration of the care manager in the care team
