Thanks to so many of you that were able to join us at the National Medical Home Summit in Philadelphia last week! The conference was just one example of how the medical home conversation is evolving to reflect its foundational role in establishing accountable, integrated systems of care. As we all know, the key features of a medical home make it possible to coordinate care across the provider continuum, manage patient populations effectively, improve the patient, family, and caregiver experience, and encourages deeper connections with community organizations and other social supports.

We look forward to highlighting this topic at our Annual Fall Conference on October 13-15th: Community Connections in the Medical Neighborhood: The Future of the Primary Care Medical Home. The conference agenda includes a range of presentations from thought leaders about the influential role of the medical neighborhood in driving health system transformation.

And be sure to dial in next Thursday, March 28th at 11AM ET for our National Monthly Briefing, where Michael P. Jeremiah, MD will discuss the Carilion Clinic's experience with medical home transformation and how it prepared them to be an ACO.

Changing the Way Medicare Pays for Care Could Save $1.3 Trillion
In a new issue brief and infographic from The Commonwealth Fund, Paying for Value, experts propose a set of policies to replace Medicare’s SGR formula with a payment system that would reduce health spending by $1.3 trillion over 10 years, offering substantial savings for state and local governments, private employers, and households. The strategies include strengthening primary care, and offering rewards and incentives for care innovations and effective use of resources.

A new Health Affairs article features a narrative from a community psychiatry researcher, and her experience receiving person-centered care for post-traumatic stress disorder and depression. The article is a thoughtful and revealing piece that emphasizes the patient's perspective, and how being treated as a whole-person can have a profound impact on health outcomes.
