Welcome to the Neighborhood: A Population Health Management Primer
Released last week, the PCPCC's latest report provides a primer for adopting a population health management approach through community-clinical partnerships in the medical neighborhood. It also provides a recommended “Top Ten List” of health IT-based population health management tools, and a series of case studies from practices throughout the country, including a pediatric practice, a community health center, and a Shared Savings Accountable Care Organization.

On November 12th, the PCPCC will host a webinar that includes presentations from Jaan Sidorov of the Disease Management Care Blog, and Robert Fortini from Bon Secours Virginia Medical Group (Tues, Nov. 12, 1:30PM-3:00PM EST, click here to register).

Oregon Gains on the Trail to Health System Transformation
Oregon continues to gain attention for its leadership in reforming health care delivery, most recently by NPR and Kaiser Health News, and their feature story about St. Charles Health System and their pilot project that embeds psychologists on primary care teams. Over two years, the therapists have cared for 400 Medicaid patients, and the model has demonstrated an 11 percent reduction in annual per patient cost of care.

According to Robin Henderson, PsyD, Executive Director of the Central Oregon Health Council, the model presents challenges for clinicians. She says, "Initially we had a couple of folks who just couldn't manage the model. It takes a different type of personality, and we find the folks coming right out of school, fresh, who've been trained in health psychology models. You have to have that type of personality who wants to engage in team-based care."

Henderson also led a workshop at the PCPCC Annual Fall Conference, which provided an overview of Central Oregon's Coordinated Care Organization. The program has implemented integrated behavioral health consultants at every level of healthcare from primary care, obstetrics, pediatrics, neonatal intensive care units and critical access hospitals, providing care to more than 35,000 Medicaid recipients.
