Delivering Quality Integrated Care with an Emphasis on
Patient and Family Engagement (PFE)

Thursday, May 25
12:00-1:00 pm ET
Addressing a person's physical and behavioral health is vital for positive health outcomes and cost-effective care. Integrated care is an effective model because it houses primary care and behavioral health services in one setting. This model also supports a team-based approach to care delivery, as it combines the various field expertise of clinicians with input from patients and their caregivers.

Join us next Thurs., May 25th at 12pm ET for a free webinar as Acacia Network, an exemplar practice in the Care Transitions PTN, shares its model of integrated care and how the patient voice has influenced effective strategies for comprehensive care delivery.
Guest Speakers:

Eva Paulino, LCSW, DBTC, Program Director, Acacia Network
Marisol Orea, LMHC, Assistant Director, Acacia Network
Elizabeth Arden, MPH, Quality Improvement Advisor, National Council for Behavioral Health- Care Transitions PTN
*This webinar is free-of-charge and is supported by Grant Number 1L1CMS-331478-01-00 from the U.S. Department of Health & Human Services, Centers for Medicare & Medicaid Services.
