Community Integrated Health in Practices: Partnerships with Impact

Successful practice transformation requires the adoption of broad population health strategies. Community-based organizations can play an integral role in this arena. As providers accept more risk under value-based payment models, there is a greater emphasis on managing chronic disease. Evidence-based programs targeting persons with two or more chronic conditions have increasing importance to success with the introduction of new payment models. 

Join us on Wed., June 21st at 1:00pm ET to learn more about how community-based organizations, like the YMCA, partner with practices to deliver evidence-based programs. 

Through testimony of a local YMCA in Seattle, Washington and their partnering practice from the University of Washington Medicine, participants will learn the steps to implementation for a sustainable clinical integration strategy.



Guest Speakers:
Tim McNeill, RN, MPH joins Y-USA as Director of Clinical Integration. In this role, Tim helps Y's position themselves as resources for navigating community health resources. He also helps guide Y's in the use of shared spaces with community health care providers.  

 
Sally Sundar, MA currently works as the Director of Health Integration and Transformation for the YMCA Association of Greater Seattle, where she directs the Association's work in formalizing operational and financial relationships with regional health care systems, insurers, employers, and other community organizations.

Participate in a Learning Network to Improve Pediatric Asthma Care
The PCPCC Support & Alignment Network (SAN) invites you to participate in a rapid cycle learning network for pediatric clinicians and TCPI practice coaches to implement asthma quality improvement activities emphasizing person and family engagement strategies.

Our quality improvement goal is to reduce emergency department visits for asthma and improve person and family engagement techniques, both important goals of the Transforming Clinical Practice Initiative.
