The PCPCC SAN and HealthTeamWorks have joined forces to present a webinar that will focus on meaningful patient engagement in transformation using a blended model to inform and improve progress toward the Quadruple Aim. Patient experience surveys at the corporate and practice level provide quantitative patient experience results that are helpful to leadership teams. By taking additional steps to engage patients at the practice level in quality improvement, richer and more timely qualitative data can be discovered that supplement and enhance the survey data.
 

Presenters:

Nicole Deaner, MSW, joined HealthTeamWorks in 2009 as a Quality Improvement Coach and now serves as the Director of Education & Training. She has worked extensively on-site with practices and systems to coach them to improve the delivery of care, reduce costs and improve the healthcare experience for patients.

Nicole now focuses her time on the needs of the healthcare workforce and building the capacity necessary to meet the needs of the increasingly complex patient population as primary care evolves to value based reimbursement. She has been a leader in designing new tools, resources, trainings and programs to support practices and their workforce both in Colorado and nationally.

 


Daniel Zirakzadeh, MBA, MBB, has over ten years of Process Engineering, focusing on improving customer satisfaction in the healthcare delivery setting. He now leads Cigna Medical Group's Clinical Operating Effectiveness team, which identifies and delivers projects that target Cigna Medical Group's quality, practice transformation, talent development, and growth & revenue strategies. 

Daniel practices a philosophy of engaging customers to drive innovation and decision-making, which has allowed Cigna Medical Group to transform its services, role structure, and culture to exceed its customers' expectation.
