Reconvening Behavioral Health Group on July 15th: Next steps and new strategic plan for PCPCC

PCPCC's Behavioral Health Special Interest Group (SIG)

Reconvening On:
Wed, July 15, 2015, 12-1pm ET

Dear Behavioral Health Members,

Over the past several months, this group has been on hiatus while the PCPCC has diligently worked to develop a new strategic plan for the organization. Setting the direction for the next three years, our new strategic plan will help us to align and engage our stakeholders to drive health system transformation. In addition, we have reorganized our stakeholder centers to enhance PCPCC’s thought leadership position by integrating the Behavioral Health SIG into our Care Delivery and Integration Center.  As one of our newest co-chairs for the Center, Polly Kurtz  of Collaborative Family Healthcare Association is helping  to lead us in this effort.

As outlined in the new plan, a key priority is to promote clinical transformation and integration into the medical neighborhood, ACOs and communities at-large. Integration of team members that support and address the mental and behavioral health needs of patients is a fundamental tenet of the patient-centered medical home. There is a strong evidence base that demonstrates that integration of behavioral health into primary care and the PCMH can lead to earlier diagnosis, treatment and better management of behavioral health conditions. Through the strategies outlined within the plan, the PCPCC will strive to further integrate behavioral health and primary care services to ultimately improve patient health outcomes, patient experience, and reduce the high costs of unmet behavioral health needs.

Join us next Wednesday, July 15th at Noon ET for a discussion with PCPCC staff, Behavioral Health advisors, and fellow SIG participants on next steps for the group.  We want to hear your thoughts and hope you can make it next week for this active dialog.  Be sure to mark your calendars!
