Population Health & Primary Care: How to Address Social Determinants Effectively  

Join us at #PCPCC2017! Primary care is both a key participant and driver of the development of strategies to improve population health by addressing social determinants, an issue increasingly being recognized as important by policymakers and payers.

Speakers:

Lisa Letourneau (Moderator), Former Executive Director, Quality Counts 

Lisa Letourneau is a physician leader and passionate advocate for health care system delivery change, with a particular interest in advancing primary care and patient engagement efforts. She previously served as Executive Director of Maine Quality Counts, a regional health improvement collaborative, where she led several quality improvement efforts including the Maine Aligning Forces for Quality initiative and the Maine Patient Centered Medical Home (PCMH) Pilot. 


Sonia Sarkar, Chief Policy and Engagement Officer, Baltimore City Health Department

Sonia Sarkar is Chief Policy and Engagement Officer for the Baltimore City Health Department. Previously, she was Director of Provider Solutions at AVIA, a health system-led innovation accelerator, and also served as Special Advisor and Chief of Staff at Health Leads, a national social enterprise that envisions a healthcare system that addresses all patients’ basic resource needs as a standard part of quality care.

Beth Tanzman, Executive Director, Vermont Blueprint for Health

Beth Tanzman is the Director of the Vermont Blueprint for Health. The Blueprint for Health reforms Vermont’s health service delivery network by developing patient-centered medical homes, community health teams, health information systems, payment reforms, and networks of community-based collaboratives.
