Opportunities to Engage with Center Leadership


During PCPCC's Annual Fall Conference in Bethesda, MD there will be ample opportunities to engage with our Center Leadership and learn more about recent and future Center activities.  Listed below are each of the Centers' Break-Out Sessions scheduled for Tuesday, Oct 15th from 10 AM - 12:15 PM.

Bridging the Gap: Alignment of State and Federal Health Initiatives

This session will include a system-wide perspective on federal health reform programs and their implications for states and local communities.


Patients, Families & Consumers

At the Heart of It All: Patients as the Key to Quality Improvement

This session will identify key strategies that will reinforce the power of patients and families in improving the health care system.


Outcomes & Evaluation

Stating the Facts: Cost, Quality, and Outcomes in the Medical Neighborhood

This session will include an analysis of the latest medical home results and research from medical home initiatives throughout the U.S.

Tracking the Shift in Benefit Design: Employers, Employees and the Medical Home

This session will bring together employers, payers, and advocates to discuss the marketplace shift toward medical home and advanced primary care employee benefit models.

(Open to All Conference Attendees)

Attendees of these sessions will have the opportunity to dialogue with leadership from all of our Stakeholder Centers. Join one of the five sessions to learn about current center projects and provide your input into future activities.  Discussions will be based around important topics and issues pertaining to each of our Centers.  Example topics include patient engagement, health IT, behavioral health, education and training, accountable care organizations, value-based purchasing, access to community resources, payment reform, etc.
