Update from Cabinet Leadership

Message from our Director of Programs, Tara Hacker.  Welcome to the December update from the Outcomes & Evaluation Center! As the new Center Lead I will be in charge of coordinating activities across our five Stakeholder Centers. I want to thank each of you for actively participating and continuing to support the advancement of primary care and the medical home model. I look forward to working with you all as this Center's work progresses.
 
Center Goal: Demonstrate the value of primary care and the patient centered medical home (PCMH), using appropriate data analysis and evaluation methods.
