Integrating Peer Support into the Primary Care Team 
This session will focus on how primary care teams are embracing Community Health Workers (CHW) to provide peer support to patients for daily management of health conditions, social and emotional needs, linkages to clinical care and ongoing availability. Panelists from both public and private health care settings will engage in a dialogue on the value of peer support, training needs, models for setting guidelines/standards, building CHWs into the care team, strategies for reimbursement and long-term sustainability.

Speaker Spotlight

Andrew Morris-Singer, MD, is the President & Principal Founder of Primary Care Progress where he has been isntrumental in igniting an interprofessional trainee-led grassroots movement to reform primary care delivery and training. Dr. Morris-Singer joins our plenary session "The Medical Home Workforce: Training in Team-Based Primary Care."
Jeanene Smith, MD, MPH, is the Chief Medical Officer for the Oregon Health Authority (OHA) and serves as the Administrator of the Office for Oregon Health Policy and Research. In her role at OHA, Dr. Smith develops medical policy and provides guidance on OHA's patient-centered primary care home program. Dr. Smith will join our plenary session "Medical Home Innovations: A View from the States."
Melissa Thomason suffered a Type I aortic dissection at just twenty-eight and was diagnosed with a rare connective tissue disorder that same year. Following her diagnosis, Ms. Thomason became a champion for patient engagement. Ms. Thomason will share her inspiring story during our Morning Keynote on Friday, November 14th.
Michael Bailit, MBA, is the President and Founder of Bailit Health Purchasing where he works with a wide array of government agencies and purchasing coalitions on payment and delivery system reform, value-based purchasing and more. Mr. Bailit will join the Outcomes & Evaluation Center breakout session to share insights on medical home evaluations.
