Much to be Thankful For...

Dear Members and Friends,

As we celebrate Thanksgiving this week, I cannot help but reflect upon our recent Annual Fall Conference and feel grateful for everyone who helped to make it a success! I want to give a special word of thanks to our patients and families who generously shared their stories with us. We heard honest stories of unbelievable courage, stories of fear, but more than anything, stories of thanks for health professionals who are willing to listen, care, and provide hope. Not one of us left the conference without feeling incredibly moved by the words of 12-year-old, Ayah – seeing the health care system through the experiences of her and her family. We appreciate all the patients and families who are willing to partner with us at the PCPCC as we work together to make improvements in health care for patients and professionals alike. 

I am also grateful for the exciting momentum behind advanced primary care, the patient-centered medical home, and health care transformation. Last week, we had an opportunity to respond to the Centers for Medicare and Medicaid Services (CMS) Request for Information (RFI) on Alternative Payment Models and the Merit-Based Incentive Payment System (MIPS). Our comments underscore the critical need for increased investment in primary care, as well as strong support for the entire care team. We invite you to read our response letter, as well as letters submitted by some of our executive members, on our MACRA resource page. 
 
As we count our blessings, I want to thank YOU for your support of the PCPCC and for your commitment to helping advance an effective and efficient health system built on a strong foundation of primary care and the patient-centered medical home. Warm wishes for a Happy Thanksgiving!

The Consortium for Older Adult Wellness, through training and consulting, connects health systems to community-based organizations in the areas of self-management and falls prevention, in alignment with healthcare transformation. 
 


The Institute for Patient- and Family-Centered Care, a non-profit organization founded in 1992, takes pride in providing essential leadership to advance the understanding and practice of patient- and family-centered care. By promoting collaborative, empowering relationships among patients, families, and health care professionals, IPFCC facilitates patient- and family-centered change in all settings where individuals and families receive care and support. It accomplishes its mission through education, consultation, and technical assistance; materials development and information dissemination; research; and strategic partnerships.



The Medi Awards works to recognize those who have served with purpose, care, compassion, and excellence while promoting the Patient-Centered Medical Neighborhood. The Medi Awards is a way to recognize people who have dedicated their time and service to improving the care of others, regardless of the capacity in which they accomplish the task.
