Newly Released Toolkit on Person and Family Engagement Culture Change

When patients and families are partners in planning and making decisions about their care, health outcomes are better, patient experience and satisfaction improves, and often, costs are lower.

Transform your practice by including the patient voice in every step of care delivery and quality improvement with the help of our new toolkit, "6 Steps to Creating a Culture of Person and Family Engagement in Health Care".

The PCPCC Support & Alignment Network (SAN) has designed this toolkit for healthcare practices participating in the Transforming Clinical Practice Initiative (TCPI). However, it is equally valuable to any practice seeking strategies to improve person and family engagement.

Jumpstart PFE in your practice by implementing these six key steps!

You may have heard about the PFE Performance Metrics and have some familiarity with it's requirements under TCPI, but what does it all mean? What activities "count" towards meeting the metrics and how can you reach these goals without adding more to your plate? We've got answers!

Join us for Part 2 in our series on PFE Performance Metrics as we present a comprehensive overview of Medication Management, Support for the Patient and Family Voice, and E-tools!

Guest speakers include:

Mary Minniti is a Certified Professional in Healthcare Quality and has over 30 years experience in the field. She joined the Institute for Patient- and Family-Centered Care in 2011 and serves as the Senior Policy and Resource Specialist.
Randy Fenninger, JD has been the Chief Executive Officer of the National Blood Clot Alliance (NBCA) located in Vienna, Virginia for the past two years. His interest in patient and family engagement began when his daughter was hospitalized for surgery and was physically lost by hospital staff between her room and the OR. It was then that he realized that patients need an advocate at all times.
Bryant Campbell has been a Patient and Family Advisor for over five years at Providence Medical Group in Portland, Oregon. He has helped develop patient-centered medical home processes, procedures, and animated media communications. In managing his chronic condition, Bryant understands the important role that his health care team, family, and community play in supporting his health goals.
