Fall-ing For Primary Care

We're back! Now that August recess is officially over, all of us at the PCPCC are looking forward to a busy fall, including PCPCC's 2014 Annual Fall Conference, which is less than two months away. Early bird registration ends tomorrowRegister today!  

With Congress adjourned and campaigning until November, most health policy issues have been put on hold including two legislative priority areas for the PCPCC, Medicaid parity extension and Sustainable Growth Rate reform. We will be pushing for action on these issues beginning the week of Nov. 10.

While the city was quieter than usual during Congressional recess, I took the opportunity to hit the road to advocate for primary care and the patient-centered medical home (PCMH). I stopped in Boston to moderate Medstro's Primary Care Innovation Challenge, a competition for innovative primary care programs. It was inspiring to see so many applicants put patients first and emphasize the value of primary care. The PCPCC chose Judy Fleishman, PhD as our top pick for her program focusing on interdisciplinary frontline staff and patient partners to improve clinic processes and communications.  Congratulations to Dr. Fleishman and her team at Cambridge Health Alliance!

We’re also excited to unveil a new feature of our Primary Care Innovations and PCMH Map. The “Outcomes View” allows users to filter program evaluation data for advanced primary care programs by outcome and payer type, state, and date of publication. The new feature also includes growing lists of aggregated PCMH outcomes data published in peer-reviewed journals and industry reports. 

August and September included a number of events and reports supportive of investing in primary care – including IOM’s report on Graduate Medical Education and Milbank Memorial Fund’s report on transforming primary care. The Bipartisan Policy Center (BPC) has been doing their part with their release of Building Better Health: Innovative Strategies from America’s Business Leaders - A Report from the CEO Council on Health and Innovation. New Map Features
Our Primary Care Innovations and PCMH Map has a new feature! The "Outcomes View" allows users to find, filter, and sort program evaluation data for advanced primary care and medical home initiatives by outcome and payer type, program state, and publication date.

The new feature also includes growing lists of aggregated PCMH outcomes data published in peer-reviewed journals and industry reports.

 
Illinois Shows Positive Outcomes 
A recent study published in the Annals of Family Medicine analyzed enrollment data from two Illinois Medicaid programs from 2004 to 2010. The programs offered enhanced fee-for-service, capitation payments, performance incentives, and practice support. Results showed:
  • $237 million in savings from 2007 to 2010
  • Reductions in utilization, with decreases in avoidable hospitalizations, overnight hospital stays, and emergency department visits
  • Significant quality improvement for nearly all metrics
  • Most prevention measures more than doubled in frequency
