Payment Reform Madness

March has been an incredible month for the movement toward more comprehensive health care payment reform. In addition to the insightful and inspiring conversations at the National Medical Home Summit in Philadelphia this week, a flurry of payment reform activity is happening in DC. 

With physicians facing a 21.2 percent cut in payments beginning April 1, the health care community has been on the edge of their seats, waiting to see if Congress will finally take action today to repeal and replace the Sustainable Growth Rate (SGR) with a fair and sustainable payment system that rewards value over volume. The draft bill represents a huge step forward and we are pleased to see the legislation recognize the patient-centered medical home as an approved care delivery model. After 17 patches in twelve years, it is time to repeal SGR, and we urge Congress to pass this bill today.

Yesterday I attended the White House launch of the Heath Care Payment Learning and Action Network which is a public and private partnership intended to support value-based payment reform. The Network was originally announced in January when the Centers for Medicare & Medicaid (CMS) set a goal of tying 30 percent of traditional Medicare payments (based on fee-for-service) to quality or value through alternative payment models by 2016 and 50 percent by 2018. We look forward to continuing to work with HHS and the White House to build a stronger health care system. 

Finally, we also had the opportunity to provide comments to the CMS on advanced primary care payment models. We appreciate the many stakeholders who helped us craft our response regarding the need to increase support for primary care that promotes value for patients, providers, and payers by tying payment reform to practice readiness for adopting advanced primary care features.

Today, the National Center and Primary Care Collaborative (PCPCC) released the first chapter in a five-part podcast series based on the PCPCC publication “Progress and Promise: Profiles in Interprofessional Health Training to Deliver Patient-Centered Primary Care,” which explored advanced primary care practices in exemplary interprofessional education teaching programs.

PCPCC Comments to CMS on Advanced Primary Care Models
On March 16, the PCPCC submitted comments to the Centers for Medicare & Medicaid Services' request for information on advanced primary care models. In the comments, the PCPCC advocates for the transition from volume-driven fee-for-service payment to more value-oriented payment models in primary care linked to quality and efficiency.

Collaboration and Practice Transformation in Residency Education 
Join the Primary Care Collaborative and Primary Care Progress on April 6 at 8 p.m. EST for a webinar on the importance of practice transformation in residency training.

The panelists will discuss how residency program collaboratives are preparing residents for future models of care delivery including patient-centered medical homes and accountable care organizations.

These collaboratives have unique goals and objectives, but are generally sharing best practices, collecting data and outcomes on new training models, coordinating learning sessions, and supporting each other through the development of new curricula for primary care residents.
