The Seventh National Medical Home Summit, co-hosted by the Primary Care Collaborative (PCPCC), will convene experts and practitioners leading the medical home movement to discuss best practices, the growing evidence base, lessons learned as well as the challenges and opportunities that lie ahead.

This year's two preconference sessions will highlight various recognition and accreditation programs as well as lessons learned from the Southeast Texas Medical Associates (SETMA) medical home transformation journey, followed by a joint plenary session with the National Population Health Colloquium. Medical Home plenary sessions will cover topics such as current medical home research, primary care workforce opportunities and challenges, population health and its connection to the medical home, the patient perspective, employer and union support of the PCMH and more. Three afternoon mini summits provide an in-depth look at clinical health coaching, PCMH implementation, and the medical neighborhood.

The medical home training program allows for a deeper dive into the subject matter and includes preconference readings, an online course, and an online exam as well as required participation in one of the preconferences and the main Summit. Candidates earn a certificate of completion with a score of 70% or better on the exam. *Additional registration fee
