January Month in Review: It's 'Snow' Lie: PCMH Evidence Piles Up!

It's 'Snow' Lie: PCMH Evidence Piles Up!
Thursday, January 28th
Dear Members and Friends,

Next week, we will uncover our latest report highlighting this year’s evidence that links the patient-centered medical home (PCMH) with lower costs and improved health care quality. "The Patient-Centered Medical Home's Impact on Cost and Quality: Annual Review of Evidence 2014-2015” is our most significant report to date, summarizing 30 primary care PCMH initiatives from around the country that measured cost and utilization of services, and – new this year – payment models. At a Capitol Hill briefing next Tuesday Feb 2nd, the Congressional Primary Care Caucus will host us for a panel discussion about the evidence and its implications – especially in light of federal payment reform and future requirements for status as a PCMH
– conversations happening right now.

Look for an email next week that will include the final publication. We encourage you to check out the report, share it with colleagues, and post your thoughts on the PCMH news. For those on Twitter, join the conversation next week using the hashtag #PCMHevidence. For those wanting to listen in on the details, join our next National Briefing webinar on Thursday, Feb. 11. And for those looking for details on PCMHs in your community/state, remember all of the studies, programs, and reports included in the publication can be found online on our Primary Care Innovations and PCMH Map.

The snow piles in DC didn’t keep us from weighing in on the Senate Finance Committee's Bipartisan Chronic Care Working Group’s policy options paper. We offered our support for Medicare policies that impact those with chronic illness, such as: improving the delivery of behavioral health services within primary care; promoting the development of aligned quality measures that are meaningful to patients; and improving access to care management by reducing out-of-pocket costs for beneficiaries – especially those who are dually eligible for Medicaid and of limited means.

And this is just the beginning. Stay tuned for more information on the Transforming Clinical Practices Initiative and our work with other CMS Support and Alignment Network partners – and thank you – as always for all you do to promote the power of primary care, our primary cause!

Stay warm!
