Assessing the Practice in Partnership with Patients & Families - Opportunities to Improve Patient- and Family-Centered Care

Redesigning your clinical practice can be overwhelming. Where do you start? How do you prioritize your quality improvement efforts? Patients and family caregivers can provide crucial insight that will help you make these decisions. Patients usually know what is working for other patients in the practice and what changes could make a big difference in making the practice more patient- and family-centered. Sometimes the greatest impacts on improving care through the eyes of the patient only requires modest effort by the practice.

*Immediately following the 60-minute webinar, there will be an optional 30-minute Q&A session with the presenters. 
 
Presenters: 

Juliette Schlucter, a Family Caregiver and Director of the Center for Child and Family Experience at NYU Langone Medical Center, has more than two decades of experience working with and leading health care professionals in patient- and family-centered care assessment, programming design and implementation. 

Patty Black, is a Program Coordinator, Patient Advisory Council member at PeaceHealth Medical Group, and faculty for the Institute for Patient- and Family-Centered Care. Upon receiving less than adequate health care that resulted in life threatening experiences, Patty has developed a passion for the patient/provider partnership and the voice of the patient.  

Mary Minniti, CPHQ, is a Senior Policy and Program Specialist, for the Institute for Patient- and Family-Centered Care. She is a Certified Professional in Healthcare Quality and has more than 30 years experience in the field. She is also the lead author on a recently published Robert Wood Johnson Foundation report, "Individual and Family Engagement in the Medicaid Population: Emerging Best Practices and Recommendations."

During this webinar, the PCPCC and the National Business Group on Health, will provide an overview of the new Issue Brief released this month: "The Primary Care Imperative: New Evidence Shows Importance of Investment in Patient-Centered Medical Homes." The brief reviews the capabilities of the PCMH model, addresses common employer concerns, and provides seven specific recommendations for employers and benefit managers regarding how they can support comprehensive primary care practice transformation and the patient-centered medical home (PCMH).

Presenters:

Marci Nielsen, PhD, MPH, President & CEO of the Patient-Centered Primary Care Collaborative, is a seasoned health policy executive with nearly three decades of state, federal, international, and academic experience, including the development and implementation of innovative policies in public and private health care programs.

Brenna Haviland Shebel, is Director of Healthcare Costs and Delivery at the National Business Group on Health. Her areas of focus include consumer-directed health care, consumerism, employee communications and engagement, and cancer. She coordinates the operations of the Institute on Health Care Costs and Solutions. 

Matt Morrison is the Executive Director of Healthcare Operations at MGM Resorts International and is responsible for leading the health care operations of the Companies’ self-funded health plans as well as providing strategic direction for and the attainment of the Companies’ goals related to employee health and wellness.  He has 15+ years of experience working with self-funded employer health plans, serving in varying capacities for both employers and administrators.
