Update from Cabinet Leadership

Message from our Director of Programs, Tara Hacker.  Welcome to the December update from the Care Delivery & Integration Center! As the new Center Lead, I will be in charge of coordinating activities across our five Stakeholder Centers. I want to thank each of you for actively participating and continuing to support the advancement of primary care and the medical home model. I look forward to working with you all as this Center's work progresses.
 
Center Goal: Across the health care delivery system, increase implementation of the patient-centered medical home, built on a strong primary care foundation, and support the development of accountable care initiatives with medical homes at their core.

Center leadership held its November conference call to further refine a work plan.  Click here to review detailed minutes from the call. 

During this call, cabinet members discussed:
  • Integration of Special Interest Groups (SIGs), Behavioral Health and Medication Management, into the work of each Center.
  • Gaps within the Center where content experts and perspectives may be missing such as the Office of the National Coordinator for Health Information Technology (ONC), Federally Qualified Health Centers (FQHCs), and Community Health Centers.
  • Previous and future webinars hosted by the PCPCC on depression screening, adolescent health, team-based care, and electronic tools to incentivize medical home care between employers, clinicians, and patients.
  • Three "Big Ideas" under PCPCC Board Review: (1) employer/provider key stakeholder meeting, (2) medical home mapper, and (3) public engagement and outreach.
