We recently opened up registration for our Annual Fall Conference, October 13-15, 2013 in Bethesda, MD. Community Connections In The Medical Neighborhood: The Future of the Primary Care Medical Home will focus on the medical home's key role within the larger health care system or 'medical neighborhood,' including behavioral health, specialty care, acute and post-acute care, public health, and community and social supports.

Each Stakeholder Center will be hosting workshops and leadership meetings designed to inform conference participants on important topics, feature experts in the field, and provide updates on Center activities.

Tracking Tool for State Innovation Models
The National Academy for State Health Policy State (re)Forum launched an online resource on the six multi-payer State Innovation Models - Arkansas, Maine, Massachusetts, Minnesota, Oregon, and Vermont. The resource includes a crosswalk of each state's:

States in Medical Home Learning Collaborative
The National Academy for State Health Policy and The Commonwealth Fund are launching a learning collaborative for states to implement multi-payer medical home initiatives. Multi-stakeholder teams from Montana, Nebraska, Pennsylvania and West Virginia will receive 18 months of individual and group technical assistance to support this work. Each state will work with multiple stakeholders to reach consensus on structural issues that are critical for multi-payer initiatives, including common definitions, standards, measures, attribution methods, and payment methodologies. This learning collaborative kicks off with a meeting this week in Denver, CO.

Europe's Experience with Medical Homes
A new study from Health Affairs: Survey Of 5 European Countries suggests that the medical home model could benefit Europe's primary care system. This study surveyed 152 primary care practices and nearly 6,500 patients in Europe and found the model particularly successful in improving care for the chronically ill.

While certain elements appear to be widespread, such as access to a physician and after hours care, some medical home features were not, such as written self-management support.

Strengthening Primary Care for Patients
On Tuesday, April 9, 9:00-10:00am ET, The Alliance of Community Health Plans (ACHP) and Kaiser Permanente will co-host this Washington, DC event featuring ACHP member plans and our CEO, Marci Nielsen, who will discuss health plan and community efforts to implement patient-centered reforms and improve the care experience for both patients and providers. This event is in response to the recent ACHP publication.

Advisory Panel on Patient Engagement
Congratulations to our esteemed COO, Amy Gibson, who was just elected to serve on the Patient Engagement Advisory Panel for the Patient-Centered Outcomes Research Institute! This panel will serve to ensure that that the highest standards of patient engagement and a culture of patient-centeredness is built into all aspects of PCORI's research and other activities.

PCPCC Webinar Tomorrow! - Register Now
Mobile e-Visits in the Medical Home: Implications of a New Delivery Model. April 4, 11:00am-12:30pm ET. Hosted by our Center for Care Delivery & Integration, this webinar will feature Dr. William Thornbury, a Kentucky primary care physician, who will co-present his two-year research findings on mobile e-Visit technology with the medical home. Other guest speakers will offer insight into patient and provider experiences with using this technology.

Call for Submissions: Innovative Residency & Training Models: Submit now to meet our April 30th deadline in order to be considered for our future publication. We're collecting residency and health professionals training models that promote collaborative, patient-centered primary care to build an online database and publication to inform policymakers and other stakeholders.

From Medical Home to ACO: March Briefing
On last week's National Monthly Briefing, the PCPCC hosted Michael P. Jeremiah, MD of Carilion Clinic, a young ACO in Roanoke, VA (presentation slides); and Deborah K. Redmond from UPMC Health Plan in Pittsburgh, PA (presentation slides). Each spoke about their organizations' experiences in creating these high-performing health systems. National Scorecard on Payment Reform

Catalyst for Payment Reform released its first ever National Scorecard and Compendium on Payment Reform. Results show that less than 11% of health care dollars pay for value-oriented care, or are tied to quality of care, performance, and/or reducing waste. The remaining 90% of payments are traditional fee-for-service; which pay providers based on quantity of tests, procedures, and visits.
