Message from the President and CEO 

Dear Colleagues and Friends,
 
With Congressional efforts to repeal or replace the Affordable Care Act in the rearview mirror for now, many of us in the policy community hope that attention will now return to efforts to derive more value for our healthcare spend. It is a focus that transcends political ideology and a foundation upon which the Patient Centered Primary Care Collaborative (PCPCC) has been built. 
 
And it is the reason why our newly released annual Evidence Report
 continues to get coverage in the news and on social media. The report contains an in-depth analysis of the performance of the patient-centered medical home (PCMH) and other forms of advanced primary care culled from a review of 1,511 journal articles and key federal and state reports. 
 
The major report findings include that advanced primary care models generally reduce costs and enhance patient satisfaction, although overall quality is mixed. This report is particularly relevant right now, as public and private policymakers continue to advance primary care models as a solution to rising health care costs.
 
A significant message from the PCPCC report is that persistence with PCMH and other advanced primary care models is necessary. Transformation will not take place overnight, and there is no one solution for every community. We need to continue our efforts to reform delivery and payment, mindful of local conditions to achieve more value for every dollar spent.

The Center for Medicare & Medicaid Innovation (CMMI) has proven itself as a crucial lever for driving such innovations, demonstrating that it can partner with the private sector and incorporate learnings to evolve their approach. CMMI has also championed investment in patient-centered primary care that needs to be sustained. The objectives of agencies such as CMMI – more value, higher quality care – are bipartisan. These are goals we should all get behind as members of Congress return from their August recess.

Michael Fine, MD

 
Michael Fine, MD, will
receive the prestigious Barbara Starfield Primary Care Leadership Award. For over forty years, Dr. Fine has devoted his career to reforming primary care to better reduce costs, improve access, and help the underserved community. Dr. Fine holds the titles of Health Policy Advisor to Mayor James Diossa of Central Falls, Rhode Island, as well as Senior Population Health and Clinical Services Officer at Blackstone Valley Community Health Care (BVCHC). Prior to these positions, he served in the cabinet of Governor Lincoln Chafee  (2011 - 2015) and as Director of the Rhode Island Department of Health.

Richard Bryce, DO

 
Richard Bryce, DO, will receive the Advanced Primary Care Practice Award for his innovative work at the Community Health and Social Services (CHASS) Center’s primary care practice. Dr. Bryce is a family physician and chief medical officer at this Detroit-based organization. As a family physician and chief medical officer at CHASS, Dr. Bryce is passionate about promoting a healthy lifestyle to his patients. He champions the Fresh Prescription Program, in which he and other providers prescribe fresh fruits and vegetables to their patients which are then made available at the clinic-based farmers market.  
 

Health Wagon

 
The PCPCC will present the Health Wagon with the Primary Care Community/Research Leadership Award, recognizing the organization’s commitment to ensuring that primary care is foundational to the community. The Health Wagon is a nonprofit organization founded in 1980 that provides mobile health services to the medically underserved in Southwest Virginia. The Health Wagon has more than 9,000 patient encounters annually for which patients receive comprehensive primary and preventive health care services. The Health Wagon has garnered notable recognition that include 60 Minutes, Nightline, CBS Nightly News, Inside Edition, Washington Post, Time Magazine and The New York Times as well as extensive international attention.

PCPCC Conference Scholarships Available!
 
The PCPCC Support & Alignment Network (SAN) is offering conference scholarships to providers, patients/patient advisors, and quality improvement coaches participating in Transforming Clinical Practice Initiative (TCPI). 

As a TCPI scholar, you will have the opportunity to dialogue with subject matter experts on implications of these innovations for person and family engagement (PFE). You will also gain access to attend our post-conference workshop that will examine application of person and family engagement (PFE) strategies with 
Choosing Wisely recommendations. TCPI is supported by the Center for Medicare and Medicaid Innovation (CMMI).

Dr. Yalda Jabbarpour, lead author of The Impact of Primary Care Practice Transformation on Cost, Quality, and Utilization, discussed the methods and results of PCPCC's newly released annual evidence report. The 2017 report highlights recently published evidence from 45 peer-reviewed reports and evaluations that link the patient-centered medical home (PCMH) and other forms of advanced primary care with improved outcomes overall, but not in every study examined. Listen to the recording.

Milbank Memorial Fund Report
 
Primary care is known to be the foundation of any high-performing health caredelivery system. But how much of our dollars go to supporting it? The Milbank Memorial Fund's report, Sandardizing the Measurement of Commercial HealthPlan Primary Care Spending, shows the importance of defining primary care spending—and provides additional questions for future research. In addition, the authors of this report present a methodology to define and estimate spending on primary care, relative to other medical expenses. Read the report on the Milbank Memorial Fund's website
