II.   How This Group Fits into Overall PCPCC Structure

A.  Organizational Structure (Attachment 1)

B.  Current Center activities/priorities pertaining to health information technology
  • Care Delivery & Integration Center to develop case studies and highlight best practices of HIT within medical home and the medical neighborhood
  • Care Delivery & Integration Center workshop at October conference on role of technology in supporting the medical home and neighborhood
C.  Procedures for communicating with Centers: Projects/tasks that this SIG identifies as a gap/need will be brought to Center Leadership by PCPCC staff (via Center monthly calls)
 
III.   Discussion of eHealth SIG Priorities

A.   Mission: To provide informal networking and educational opportunities related to the integration of HIT into patient-centered medical homes and the medical neighborhood.

B.   Group Makeup - open to all interested in topic - Clinicians, Health Plans, Health Care Practices, HIT Vendors, Health Policy Analysts, Professional Associations, Government Employees, etc.

C.   Ideas for Future Calls: Could include invited guests; “hot topic” agenda items; program updates from the field; announcement of news & resources; etc.

D.   Future Agendas: Staff will work with SIG leaders to develop; if any SIG member has an item they would like to discuss, please email Tara Hacker at
thacker@pcpcc.net by COB the Thursday before the call.

E.   Procedures for Communicating amongst SIG members: Developing an online chat forum accessible through this
group’s webpage (example from PCPCC’s Behavioral Health SIG -  "Oregon's Health Reform Experiment")
 
REMINDER: Our next eHealth SIG call will be June 4th (Tuesday), from 12-1pm ET. This group has a standing call on the first Tuesday of the month – same time, same call-in number/access code.
