Our panels this year will cover a wide range of topics including practice transformation, the primary care workforce, state perspectives on health reform, federal health care reform and community partnerships. Learn more about our panel discussions here.

4. More Practice Transformation Educational Sessions:
This year's conference gives attendees more opportunities to engage with practice transformation thought leaders and experts from the field. Don't miss our panel, which will feature stories and lessons learned from providers and patients across the health care spectrum, or our two breakout sessions on Self-Guided Practice Transformation and Practice Coaching this year.

3. Three Inspiring Keynote Presentations: 
We are delighted to have Kathy Greenlee, Administrator for the Administration for Community Living and Assistant Secretary for Aging at the U.S. Department of Health and Human Services deliver the opening keynote on Wednesday, Nov. 12th. Thursday's plenary will begin with a keynote from 
Mark McClellan, MD, PhD, Senior Fellow and Director of Health Care Innovation and Value Initiative at the Brookings Institution. Melissa Thomason, patient and family advisor, joins us Friday morning to share insights on the patient's perspective in health care. 

2. Greater Focus on State Medical Home Activity:  
As more of health delivery reform happens at the state and local level, we are pleased to offer attendees two opportunities to learn about strategies states are using to advance the medical home. Be sure to check out our State Innovations panel for a look at the work being done in Minnesota, Oregon and Colorado and our State Collaboration breakout session, where Ohio leaders will share their success in bringing together multiple stakeholders to achieve better health for Ohioans.

We invite you to join PCPCC leadership, staff and friends at the
PCPCC's Inaugural Awards Dinner where we will honor Debra L. Ness, MS, 
Ted Wymyslo, MD and the Southeast Texas Medical Associates for their special contributions and dedication to the promotion of primary care and the medical home.

Speaker Spotlight
Jennifer Baldwin, RN, MPA is the Vice President of Patient-Centered Medical Home Regional Care Coordination at CareFirst. Ms. Baldwin joined CareFirst from Kaiser Health Plan where she was the Area Administrator for 13 primary and specialty care facilities in Washington, D.C., and Maryland. Ms. Baldwin will offer insights on our Beakout Session "Go (Primary Care) Team: Team-based Care in the Medical Home."
Christopher L. Hunter, PhD, ABPP, works for the Defense Health Agency as the DoD Program Manager for Behavioral Health in Primary Care. He has extensive experience training individuals to work in behavioral health clinics and primary care settings treating common mental health conditions, health behavior problem areas and chronic medical conditions. Dr. Hunter joins us to participate on our Breakout Session "Practicing Whole-Person Care in the Medical Home."
