8. Strengthen Patient, Family and Clinician Partnerships
New to our conference this year is a special Patient, Family and Clinician Partnership Training. Learn strategies and best practices for integrating patient- and family-centered care into your practice’s quality improvement and care redesign efforts; build upon your experiences as a patient partner or receive the tools needed to start a program at your practice; and connect to opportunities to help local clinicians improve the way they are providing health care services. Participants will work directly with expert faculty from the Institute of Patient- and Family- Centered Care (IPFCC), Planetree, and the YMCA of USA to develop, implement, and evaluate strategies used to drive practice transformation.

7. Get the Lay of the Election Landscape (& What It Means to Primary Care)
Just days after the election, be among the first to learn what the results will mean for primary care. During the panel "Primary Care & Policy: Post-Election Analysis," health policy experts from both sides of the aisle will prognosticate on the post-election environment for continued delivery and payment reform.

6. Hear a Payment Reform Progress Report  
With the new Medicare payment plan expected to be finalized in November, hear an insightful panel discussion to learn about the scale and spread of payment reform efforts that move us from volume-based to value-based payment, including the federal Comprehensive Primary Care Plus initiative, the Medicare Access and CHIP Reauthorization Act (MACRA), and other private sector payment innovations.

5. Learn the Keys to Community Engagement

Putting Best Practices into Community Playbooks: This panel will share best practices from across the country that have proven successful in clinic-to-community partnerships and offer insight into how payment reform can help to incentivize these efforts in other communities, regions, and states.



4. Hear About the New Administration's Health Care Innovation Agenda  
Esteemed journalist, health policy analyst, and President & CEO of the Network for Excellence in Health Innovation Susan Dentzer will offer keen insight on the future of primary care transformation & the “quadruple aim” as the next Administration (and Congress) take over the federal policy leadership reigns. 3. Prioritize Primary Care Spending  
If primary care is so important, why do we know so little about how much of our health care dollars go to support it? And what happens to expenses and population health if we actually try to spend more on it? Christopher F. Koller will talk about recent statewide and national efforts to measure the portion of medical expenses going to primary care and what happens in the delivery system when that portion is increased. See keynote speaker spotlights below for further details!

2. Set the Stage for the Shared Principles of Primary Care
Together, the PCPCC and FMA Health are engaging a diverse set of stakeholders to identify a new set of shared principles to unify and reinvigorate the primary care movement. Join us November 11th (the day after our Annual Conference) at the Summit on Shared Principles of Person-Centered, Team Based Primary Care to help determine the future agenda of the PCPCC and this exciting multi-stakeholder movement.

Keynote Speaker Spotlight
The Health Care Innovation Agenda for the Next Administration

Susan Dentzer, a leading national expert in health care and health policy, is the President and Chief Executive Officer of the Network for Excellence in Health Innovation. In her role as President and CEO, Dentzer drives the organization’s strategic agenda and priorities, works closely with its members to identify key issues, promotes NEHI’s research and policy recommendations, and serves as the organization’s spokesperson, providing insightful analyses of the U.S. health care system.

Christopher Koller is president of the Milbank Memorial Fund, a national operating foundation that improves population health by connecting leaders and decision makers with the best evidence and experience. Before that, he served in the State of Rhode Island from 2005 to 2013 as the nation’s first health insurance commissioner, where he and his team aligned regulation and commercial health insurance rate review with statutory authority to help promote delivery system reform, in such areas as primary care transformation.
