October is a big month for primary care! Join us for Patient-Centered Care Awareness Month as we honor primary care providers and the patients and families they serve with a presentation from Planetree. We will also celebrate National Primary Care Week and National Health IT Week Oct. 5-9 with a special joint presentation by executive leaders from HIMSS and URAC. As we pay heed to primary care (as we always do at the PCPCC), we are pleased to host two webinars in observance of these special events. Telehealth in Primary Care:
Increasing Access and Integrating Care

The PCPCC, in partnership with HIMSS and URAC, is pleased to host this webinar that will explore the impact of connected health technology on the daily workflow of healthcare providers and will highlight how HIMSS and URAC are addressing issues associated with the broadening definition of telehealth.

Thomas R. Martin, PhD, MBA, is Director with HIMSS, a global, cause-based, not-for-profit organization focused on better health through information technology (IT) supporting the organizations focus on mobile, wireless, and telehealth technologies.

Kylanne Green is President and CEO of URAC, a national accreditation leader. A Registered Nurse and Adult Nurse Practitioner by background, Ms. Green has more than 40 years of experience in health care delivery and insurance strategy and operations.

The Planetree Framework for Patient-Centered Excellence

Long before patient-centered care became a centerpiece of health care reform efforts or a media buzz word, Planetree was defining what it means to be patient-centered. With more than three and a half decades of experience listening to patients and partnering with organizations to design and implement patient-centered approaches to care, the Planetree model provides, in concrete and actionable detail, a pathway for: 
  • Partnering more effectively with patients 
  • Engaging family members to improve quality, drive outcomes and foster continuity 
  • Creating workplaces that promote superior performance, and 
  • Redesigning organizational structures around the patient 
This webinar will provide an overview of the theory behind the Planetree methodology and will introduce the Patient-Centered Designation Program as a framework for accelerating patient-centered transformation and improving outcomes.
 
Presenter:
Sara Guastello, is Director of Knowledge Management at Planetree where she has developed a range of educational and implementation resources to guide organizations in their endeavors to deliver patient-centered care. She oversees the Patient-Centered Hospital Designation Program® and the associated Patient-Centered Merit Recognition Program, the only such program to recognize excellence in patient- and resident-centered care across the continuum of care.
