The next PFCC.Connect virtual informal conversation will be held on Wednesday, May 1, 2019 at 10:30 AM ET/7:30 AM PT on Medication Management.

Medication Management is an issue that impacts health outcomes and safety. Literature on the topic highlights the disconnect that can occur between the healthcare team and the patient goals. For instance, patients do not take their medicine as prescribed 50% of the time. This number may be as high as 80% for patients with many chronic illnesses, especially for those who are asymptomatic. Over 25% of initial prescriptions are never filled. For diabetes care, this may be as high as 40%.

What are ways your organization collaborates with patients and families in direct care to support their medication management? How have you invited patients and families to partner with you to address approaches/systems to improve medication safety? Bring your ideas, questions, and strategies to our conversation!

Register for the discussion here.

If you are interested but unable to attend live, I still encourage you to register because all registrants will receive a link to the recording of the discussion after the event. The recording and any resources will also be posted in PFCC.Connect for all members to watch and/or refer back to. If you have further questions, please email me at alee@ipfcc.org or Mary Minniti at mminniti@ipfcc.org. I hope to talk with you next Wednesday!
