Three Strategies for Effective Referrals to Specialty Mental Health and Addiction Services

Mercoledì Ago 19, 2015 02:30 pm to 04:00 pm EDT
Stati Uniti

Integrating behavioral health doesn’t mean primary care providers can support all the behavioral health services their patients may require. Linkages to specialty mental health and addiction treatment and rehabilitative services will always be necessary.  Effective referral relationships are critical for safety-net providers, especially those identified as Patient Centered Medical Homes and providers who adopt screening, brief intervention, and referral to treatment (SBIRT).

Join this webinar to hear from three providers, each who successfully implemented a different strategy of referral to specialty care. Learn how these providers foster community partnerships to effectively meet their patients’ needs for intensive mental health and addiction treatment.

After this webinar, participants will:

  • Understand how effective referrals fit into Patient Centered Medical Homes and SBIRT models
  • Identify four strategies for forming partnerships with specialty mental health and addiction services for effective referrals
  • Recognize practical tips and resources to help establish appropriate referrals.
