North Dakota

Medical home efforts in North Dakota have been led by the private sector. In 2009, Blue Cross Blue Shield North Dakota (BCBSND) launched MediQHome, a statewide program to provide enhanced care coordination and chronic condition management in primary care practice. More than 72% of primary care providers across the state participate in this program. On October 1, 2011, North Dakota Medicaid began transitioning their disease management program to a Health Management program. Services of this program include patient-centered care coordination, telephone contact with a care coordinator or nurse within the clinic, involvement from a team of medical professionals as needed, and patient education. Providers, pratitioners, clinics, or Disease Management Organizations (DMOs) may be eligible to provide additional health management services to certain, eligible Medicaid recipients with chronic diseases. North Dakota also has an Integrated Services for Children and Youth with Special Health Care Needs grant that is working to provide significant education, training, and opportunities for primary care providers, families and other important partners in coordinated care.
