To implement PCMH principles, the Bureau of Medicine and Surgery (BUMED) formally developed its PCMH model Medical Home Port (MHP) and is in the process of implementing MHP across its enterprise. Medical Home Port helps provide primary care in a way that best meets the needs of Navy Medicine’s beneficiaries. The Medical Home Port team ensures that care is all-inclusive and integrated with all other care provided within the healthcare system. Care delivered in Medical Home Port model includes, but is not limited to, readiness, prevention, wellness, behavioral health, and disease management. 

Active-duty service members and their families are automatically enrolled in TriCare Prime, a health care program that costs them nothing and covers everything. Under the Navy’s Medical Home Port, each patient is assigned a team led by a provider. The patient is a part of the team that also includes a nurse educator, care coordinator and other support staff.
