Description: 

The REACH Foundation Medical Home initiative began in 2007 and is focused on supporting safety net health care clinics (FQHCs, pediatric-focused, nurse-led, “free” health clinics and other community health centers) in adopting patient-centered medical home practices using the National Committee for Quality Assurance (NCQA) PCMH framework. The REACH Foundation provides participating clinics with individualized technical assistance and supplemental grants to support implementation of key components of the medical home, such as Electronic Medical Records, care coordination, quality improvement monitoring, and improved staffing models.

Since 2011, six clinics have achieved Level III NCQA PCMH recognition. The initiative is ongoing with new emphasis on integration of oral health and primary care and behavioral health and primary care, as well as ongoing support for transformation efforts.
