Description: 

The Rainier Health Network (RHN) is the second CMS-designated ACO in Washington State.  The CMS contract covers the management of care for 27,000 Medicare FFS beneficiaries whose care is attributed to the 600 providers participating in the Rainier Health Network.  The goal of the contract is to reduce the cost of care for these beneficiaries by improving the coordination of care, reducing unneeded tests and procedures, and working with patients to help them manage existing chronic conditions more effectively. 

Payment Model: 

The contract is called a Medical Shared Savings Program, which is designed to split the savings between the ACO and CMS if basic contract targets are achieved.

Cost Savings: 

First Agreement Period:

  • Shared Savings for Performance Year 2015, $2,981,335
  • Proportion invested in infrastructure: 20%
  • Proportion invested in redesigned care processes/resources: 20%
  • Proportion of distribution to ACO participants: 60%

 

Increased Preventive Services: 
  • Preventive Care and Screening: Tobacco Use: Screening and Cessation Intervention: 93.43%
