Description: 

Primary Blue focuses on chronic disease management (diabetes, congestive heart failure, coronary artery disease, hypertension, metabolic syndrome and asthma), as well as preventive services (BMI recording, colorectal cancer screening, breast cancer screening, cervical cancer screening, and immunizations). The three types of providers participating in Primary Blue are: Family Physicians, Internal Medicine Physicians, and Pediatricians.  Blue Cross and Blue Shield of Nebraska is recognizing physicians who are participating in the Primary Blue Patient Centered Medical Home with a BPR indicator.  As of April 2013, more than 330 physicians are participating in 15 Nebraska communities: Auburn, Bellevue, Crete, Elkhorn, Grand Island, Gretna, Kearney, LaVista, Lexington, Lincoln, Nebraska City, Omaha, Papillion, Valley and West Point.
