In February 2010, Maine and Vermont were awarded a 5-year demonstration grant from the Centers for Medicare & Medicaid Services, authorized by 401 (d) of the Children's Health Insurance Program Reauthorization Act of 2009 (CHIPRA).

The project focuses on using quality measures and information technology to improve health outcomes for children. The goal is to improve timely access to quality care for children who are insured by Medicaid. The State of Vermont is working to: 

  • Expand and support use of the Vermont Blueprint for Health central clinical registry (Covisint/DocSite) to support care in the pediatric population.
  • Extend the Blueprint Advanced Primary Care Practice model to child-serving practices by supporting National Committee for Quality Assurance (NCQA) Patient-Centered Medical Home assessments and practice facilitation for ongoing quality improvement.
  • Expand the number of states with an Improvement Partnership program and evaluating the model as an effective strategy for improving the quality of children’s health care.
